SMRMC and all associated clinics will transition to a new electronic health record known as Epic with a new self-service patient portal, MyChart, on JUNE 1, 2023 after whichtime you will only be able to access Follow My Health for results and notes from previous appointments.

MyChart provides you with online access to your medical records. It can help you become an active participant in your healthcare and communicate quickly and more effectively with your providers.
